Carers

  • Are you a Carer? Let us know!

Are you a Carer? We would like to identify all the patients that are carers or have carers (regardless of age). We want to ensure that all patients are getting the right help and support.

A carer is someone at home or in the family, a friend or someone in the neighbourhood that you depend on to help with tasks and responsibilities of everyday living, and is not employed to provide care.

Please download and complete our form (PDF) or contact Steph (our carers’ champion) at the surgery.

The surgery works alongside Carers Milton Keynes who specialise in supplying advice and support to carers of all ages, from the age of 8 years right through to and including adults. We can provide you with the appropriate carers pack and further information.

Alternatively, you can contact them direct via their website on www.carersmiltonkeynes.org or telephone 01908 231703.
